NEW PREQUALIFICATION APPLICANTS

Before you get started you will need the following information to complete the application:

Copy of W-9
List of company license numbers
List of state sales tax numbers
List of state unemployment insurance numbers
List of insurance agreements
List of current projects
List of recently completed projects
Current financial statement
Bank information

Dun and Bradstreet information
Surety information
3 supplier references
3 contractor references
Independent verification letter supporting
your EMR for last 3 years
OSHA 300 logs from last 3 years
Insurance information

PROGRAM SUBCONTRACTING REQUIREMENTS

Aviation Owner Controlled Insurance Program (OCIP) includes coverage of:

On-Site General Liability Insurance
On-site Excess Liability Insurance
Pollution Liability
Builder’s Risk

The Subcontractor is still responsible to provide evidence of off-site insurance coverages including but not limited to Automobile insurance, Professional Liability insurance and Workman’s Compensation insurance (both on and off-site), etc.

Project Requirement of 25% LDBE Participation
(Local Disadvantaged Business Enterprise)

To become an LDBE, the application form is available for download on MWAA’s site: www.mwaa.com/contracting

All awarded subcontractors will be required to submit a Letter of Intent at time of subcontract award for their for their LDBE subcontractors and regularly submit an MWAA LDBE/MBE/WBE Contract Participation Form

Minority and Women Business Enterprises (M/WBEs) participation goal is Fifteen percent (15%)of the total contract value.
